This past week an issue developed that you need to relay to our members.
Earlier in the year the Company sent out a "Positive Enrollment and Emergency Contact Forms for Unionized Employees". Some employees did not receive this form, others did not realize the importance of submitting the form and the form is confusing in that it looks like you only have to submit it if there are any changes to you status.
In the documentation the Company noted that if the form was not submitted the employee would be considered single. The original deadline was set back from April 2001 to October 2001. The effect of this caused the employee's spouse and dependants not being covered by the benefit plans (dental and health ...) and claims returned to the employee not paid.
Employees can find out what their status is by calling staff records clerk at the CMC or HR help desk at 1-866-319-3900.
Dennis is in the process of talking with Keith Wheeler about the problems, why it happened and ways to fix this, but in the mean time it is important that employees, with spouses and/or dependants, make sure that the form was filled out and sent in to the Company.
